Women who want to attend Aventa’s programs will be asked to complete our online application form or download and print an application form which can be faxed, emailed or mailed to Aventa. Our fax number is (403) 245-9485. After sending us the application, please follow up with a phone call to ensure that we have received your application and to book an assessment appointment.

Assessments can be done by phone, Zoom or in person. When completing a phone or Zoom assessment, please ensure you are able to call or connect from a quiet, confidential, and safe location (i.e. not while driving or in a public place). Assessment appointments take approximately 1 hour and you must not be under the influence of drugs or alcohol during the phone or on-site assessment.

Once the assessment has been completed, the Assessments Counsellor will determine the most appropriate level of care for you, either at our facility or elsewhere. If the Counsellor recommends treatment at Aventa, they will book a date for you to come in for treatment, and give you a list of what to bring.

You may call the Assessments Counsellor if you have any additional questions prior to your arrival. If you are unable you reach us during business hours Monday – Friday 8:00 am – 3:00 pm, please leave us a message and we will get back to you as soon as possible. You may also contact Aventa by email at info@aventa.org.

Due to the vulnerabilities that come with a congregate setting/shared environment and for those experiencing possible health concerns, it is strongly encouraged that eligible individuals get vaccinated prior to attending treatment. “Vaccines are a critical way to limit the spread of COVID-19. They are effective and safe. Immunization protects your health, as well as the health of your loved ones and the community.” – AHS. If you are interested in getting your COVID-19 vaccine, you can book via a local participating pharmacy, by calling 811 or online through AHS Immunization Booking.
